Reentry Community Health Worker
Job Title- Reentry Community Health Worker
Description
The vision of a Community Health Workers (CHWs) is to provide a holistic recovery model for Justice-Involved individuals reintegrating into the community from the Los Angeles County Jail or California State Prison system.
Responsibilities
- Provide high-quality case management to individuals re-integrating into their communities from the justice system.
- Develop care management teams that include community health workers, CSWs, and administrative support staff.
- Have a client-centered approach and provide excellent customer service that is sensitive to the challenges that Justice-Involved Populations face reintegrating into their communities, especially navigating complex medical, behavioral, and social service delivery systems to address chronic health and behavioral health conditions.
- Other duties as assigned.
- Employ a “whatever it takes approach” to assist clients in their transition from Jail or Prison to community reintegration and stabilization.
- To engage in community outreach.
- To attend all scheduled CHWs training.
Qualifications
- It is a desirable qualification that Community Health Workers (CHWs) possess “lived experience” to meaningfully connect with clients.
- CHWs are to have been incarcerated for a continuous period of six months, provided that such individuals have not been incarcerated in the past twelve months and are not under active or mandatory supervision by a Federal, state, or county probation or parole department in the past twelve months.
- Previous experience working with system-impacted communities, including but not limited to people who are incarcerated and/or formerly incarcerated and communities of color.
- Demonstrated ability to work well independently and pursue opportunities for self-teaching and skill development as needed. The ability to build productive and collaborative relationships with organizational partners.
- Must have reliable transportation source.
- Ability to work independently as well as with a team of caring and motivated professionals.
